Abstract

Living Wills are a document that aims to ensure the right of patients to exercise their will in the end of life. Throughout history we have seen that the living will must not become an objective but a means. It is essential to include this document in a broader and comprehensive framework for action, the Advance Care Planning so that we can really help ensure patient autonomy at the end of life. Primary care is the level where this process must be done. Community Nursing is the profession who must exercise the leading role in the Advance Care Planning. Community Nursing, by knowledge, by methodology, by skills, attitudes and professional values ​​is the collective that bring added value to the process of advance care planning. The Administration should promote the implementation of this process by promoting the training of health professionals, facilitating the process of granting and registration of the document to citizens; and improving way to access the contents of the document from electronic health history to enforce that right.
